1.The undersigned individual agrees that Penn Pride, Inc. will in the future supply all heating oil to the address shown below. The automatic deliveries will be scheduled by Penn Pride, Inc. Once the undersigned agrees to this contract, no heating oil deliveries shall be made to this property other than those made by Penn Pride, Inc. Any outside deliveries would interrupt the automatic calculation and could possibly result in over filling the fuel tank and spilling heating oil, or in completely running out of heating oil. The heating oil deliveries to this property will continue year round until Penn Pride, Inc. receives written notification to cancel the automatic delivery.

2. The undersigned agrees to supply credit card (NOT debit card) information that will be kept on file and used to pay all invoices upon receipt of each oil delivery. The terms for all Penn Pride, Inc. heating oil deliveries are COD (cash on delivery). The credit card listed on the account will be charged immediately after the tank is filled. 

3. All balances, including finance charges, which are over thirty days past due shall be subject to a finance charge of two percent per month. If the undersigned fails to make any payments when due, the undersigned hereby authorizes and empowers the prothonotary, clerk of court, or any attorney of any court of record to appear for and confess judgment against the undersigned in favor of Penn Pride for the total sum due, as of any term at any time or any number of times, with or without defalcation, with interest and costs and with costs added for collection of fees and attorney fees equal to ten percent of the total sum due. Stay of execution, inquisition and extension upon any levy of real estate is hereby waived and condemnation agreed to. The exemption of personal property from levy and sale on any execution hereon is also hereby expressly waived and no benefit of exemption be claimed under and by virtue of any exemption law now enforced or which may be hereafter passed. Failure of Penn Pride to exercise this right at any particular time does not constitute a waiver of the right. The undersigned agrees that he/she will be held financially liable for any and all heating oil purchases.
